NEW MEDICAL PATIENT ELIGIBILITY Before you apply to receive treatment at Marian Clinic Medical, you should know: Marian Clinic is not a free clinic. Once you have established eligibility you will be charged a fee. The fees are $15 per appointment. Marian Clinic Medical is not an emergency room or walk-in clinic. If you have a life-threatening emergency, please go to a hospital emergency room. The hospital’s policies and fees will apply. You may not be seen immediately. The entire process from when you first fill out an application until you have your first doctor appointment may take from four weeks to two months, depending on how thoroughly you provide us with the information needed to qualify you as a patient and when a doctor is available that fits your health situation. Marian Clinic does not accept private insurance. Medicare, Part A is considered to be insurance. If you are receiving Medicare, Part A, you are not eligible for Marian Clinic Medical services. Seniors eligible for Medicare are not eligible to become Marian Clinic patients. This applies to persons whether or not you have actually applied for Medicare. Marian Clinic does not treat Workers Compensation injuries or injuries sustained in a motor vehicle accident. (This is covered by insurance.) Some veterans receiving services from the Veterans Administration may still qualify for services at the Marian Clinic Medical, provided you meet other Marian Clinic Medical eligibility requirements. Business hours are Monday – Thursday 8:30 a.m. to 12:00 p.m. and 1:00 p.m. to 4:30 p.m. and Friday 8:30 a.m. to 12:00 p.m. and 1:00 p.m. to 3:30 p.m. Failure to answer all questions or to provide/attach complete information and proof of eligibility will delay your processing and delay you receiving an appointment at the Clinic. 4. An Eligibility Screener will review your information. If your information is not complete, lacks proof of income, or if we have questions about your application, we may call you on the phone. You may be asked to answer additional questions or to bring in additional proof of eligibility. Please make sure the phone number(s) you provide allows incoming calls. 5. Once your application to become a medical patient has been approved we will notify you by letter. The letter will contain the date and time of your first Marian Clinic doctor appointment.
